Apply for a health scholarship

The Suz Fund offers health scholarships for people undergoing or recovering from cancer treatment and/or diagnosis to offset some of the financial burden and to help people pursue the specific treatment or wellness support they want or need.

To apply for a scholarship, fill out the form below, which requests that you:

Provide a brief written statement about yourself and your cancer - as your time and energy allows. Be as brief as you would like - your job is to heal and this shouldn’t be another burden.
Provide a short description of the wellness support you are seeking, who would provide it, and how much it would cost (rough estimate okay).
Please do your best to enjoy the process of explaining your vision without it creating any stress.

The Suz Fund offers health scholarships at the end of each quarter (every 3 months), so scholarship funds will be given ~ March, June, September, and December! Funds will go directly to a provider or business - of the applicants choice - to cover up to $500 of those costs. Thank you for allowing us the opportunity to help.

APPLICATION PERIODS for EACH YEAR:

1st quarter: application period ends March 15th - applicants contacted by early April*

2nd quarter: application period March 16th-June 15th - applicants contacted in early July*

3rd quarter: application period June 16th-September 15th - applicants contacted in early Oct*

4th quarter: application period Sept 16th-December 15th - applicants contacted in early Jan*
